The role you’ll contribute:

The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources and qualify for community assistance from a variety of special assistance programs and agencies, and/or require assistance with transitions of care or discharge planning. This role involves crisis intervention, developing discharge plans for high-risk patients, and collaborating with interdisciplinary team members to ensure patient-centered care and efficient resource utilization. Responsibilities include patient evaluations, development of transition of care plans, ensuring smooth patient flow, and educating team members on resource utilization and care coordination. The Social Work Care Manager also assesses post-hospital needs, manages barriers to care, and facilitates communication among care team members to optimize patient outcomes and safety.

The value you’ll bring to the team:

• Psychosocial Assessment and Interventions: including risk factor assessment, emotional and social support, grief and crisis counseling, advocacy, motivational interviewing, and patient/family education.
• Providing assessments and reporting interventions in cases of abuse, domestic violence, neglect, and trafficking.
• Consulting on decision-making capacity, guardianship, foster care, and mental health assessments.
• Facilitating ethical discussions, promoting advanced directives, and documenting care goals.
• Conducting initial evaluations within one day of admission, reviewing medical and social information, and incorporating patient preferences into care plans.
• Collaborating with the interdisciplinary team to resolve barriers, monitor readmission risks, and ensure timely care transitions.
• Preparing discharge plans, managing post-hospital services, and ensuring compliance with documentation and regulatory requirements.
• Educating team members on payor requirements and maintaining current knowledge of community resources and post-acute care options.
• Participating in quality improvement activities, department training, and serving as a preceptor.
