Schedule: Part-time Saturday 7a-7:30p and Sunday 8a-4:30p; Holiday rotation

The role you’ll contribute:
The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources and qualify for community assistance from a variety of special assistance programs and agencies, and/or require assistance with transitions of care or discharge planning. This role involves crisis intervention, developing discharge plans for high-risk patients, and collaborating with interdisciplinary team members to ensure patient-centered care and efficient resource utilization. Responsibilities include patient evaluations, development of transition of care plans, ensuring smooth patient flow, and educating team members on resource utilization and care coordination. The Social Work Care Manager also assesses post-hospital needs, manages barriers to care, and facilitates communication among care team members to optimize patient outcomes and safety.

KNOWLEDGE AND EXPERIENCE STRONGLY PREFERRED:
