Full-Time: 40 Hours Per Week

1st Shift: 8am - 4:30pm
Monday - Friday
Remote with Onsite Requirements as Needed

Summary:

The SW Care Manager Plus, under the direction of the department Supervisor, applies graduate-level clinical social work skills for the deliberate organization of patient care activities between two or more participants (including the patient and/or family) involved in a patient's care to facilitate the appropriate delivery of health care services. Coordinates the delivery of care in collaboration with the multidisciplinary healthcare team within the practice setting and across health care settings to assess, evaluate, screen, and develop action plans to mediate gaps in care.

Responsibilities:
  1. Assessment: Systematically collects comprehensive and focused data relating to health needs and concerns of a patient, group, or population as they move across the care continuum.
  2. Outcomes Identification: Analyzes assessment data to determine patient/family/caregiver needs and facilitate the appropriate services needed across the care continuum. Identifies expected outcomes specific to the patient, group, or population.
  3. Planning: Develops a patient- and/or population-centered plan of care that identifies strategies and alternatives to attain expected outcomes based on funding resources, services, clinical standards, and outcomes. Provides consultation to influence plans of care and enhance the abilities of other professionals.
  4. Implementation: Implements the identified care plan to attain expected outcomes. Uses educational strategies for patients, families, and healthcare team members about treatment options, resources, and psychosocial concerns. Maintains objectivity and adheres to timelines and department standards.
  5. Evaluation: Evaluates patient, group, or population progress towards outcomes and communicates findings to relevant professionals.
  6. Demonstrates understanding of care management, complex disease management, transitions of care, post-acute care, and community management standards. Provides comprehensive care addressing physical, psychological, social, educational, and safety needs of patients of all ages.
  7. Performs other duties as required.
Other Information:
Technical Expertise
  1. Experience with healthcare coordination in managed care, homecare, or community agencies is preferred.
  2. Requires critical thinking, clinical judgment, strong communication, organizational, and leadership skills.
  3. Demonstrates professional, effective, and tactful communication skills, including written, verbal, and non-verbal.
  4. Proficiency in MS Office (Outlook, Excel, Word) is required; Epic or similar EMR software is preferred.
Education and Experience:
  1. Master's Degree in Social Work from an accredited program with a current unrestricted license as a Licensed Social Worker (LSW) in Ohio.
  2. Minimum of 3 years of clinical healthcare experience; pediatric ambulatory or care management experience is preferred.
  3. Certification by a recognized care management organization is preferred.
  4. No supervisory experience required.
