Job summary

Creedmoor Psychiatric Center is seeking a Social Work Assistant 1 to provide case management and service coordination for individuals with mental illness. You will work with an interdisciplinary team to interview individuals and families, prepare histories, and determine eligibility for services.

Role involves advocating and helping clients access needed services, with opportunities for growth within OMH’s team and programs. Commitment to professional standards and collaboration is essential.

Qualifications

• Bachelor’s degree in a human service field and 1 year of social work experience.
• MSW can substitute for 1 year of specialized experience.
• State Service transfer eligibility if already in title eligible for transfer.

Responsibilities

• Interview individuals and families to gather background information.
• Prepare case histories and determine eligibility for services.
• Advocate and assist individuals in identifying and obtaining services.
