Job summary

InnovAge Holding Corp. seeks a Social Work Assistant to support program participants by coordinating essential services and benefits. The role includes helping with Medicaid/Medicare applications, housing referrals, and documenting needs with the Social Worker team.

You will research community resources, verify participant data, and assist with outreach and referrals to ensure participants receive timely support. Travel within the service area may be required.

Qualifications

• Community Health Worker training or certification course.
• Associate’s degree in healthcare, social services, or equivalent.
• Bachelor’s of Social Work, Health Care Administration, or equivalent preferred.
• Require personal transportation, current state issued driver’s license, good driving record and auto insurance.
• CPR/BLS and First Aid required in California only.
• Two years’ experience in community-based social services or healthcare setting.
• One year experience with older adults and people with disabilities.

Responsibilities

• Contact participants to confirm delivery of DME and other interventions.
• Assist participants with Medicaid/Medicare applications and recertifications.
• Help housing, homelessness prevention, food security or related programs as needed.
• Research and catalog community resources and referral contacts.
• Locate and verify participant locations, numbers and emergency contacts.
• Update demographic information in InnovAge systems as required.

Education

Community Health Worker training or certification course

Associate’s degree in healthcare, social services, or other equivalent training

Bachelor’s of Social Work, Health Care Administration, or other equivalent training
