Job summary

Georgia Pines Community Service Board seeks a Case Manager to join the Outpatient Mental Health Co-Responder program in South Georgia. The role focuses on post-crisis outreach, coordinating services, and engaging with community resources, while ensuring proper documentation and program accountability.

The position covers Colquitt and Mitchell Counties, enabling efficient operation and strengthening collaboration with law enforcement and clinical teams.

Qualifications

  • High school diploma or GED and two years' experience in a social service-related position.
  • Bachelor's degree in a social service-related field.
  • One year at the lower level or position equivalent.

Responsibilities

  • Focus on post-crisis outreach, coordination of services, and engagement with community resources, while supporting documentation and program accountability.
  • Covering Colquitt and Mitchell Counties to support the co-responder model and collaboration with law enforcement and clinical teams.
  • Report to the Colquitt County Co-Responder Team Lead with appropriate latitude for using seasoned judgment.

Education

Georgia Pines Decatur County Mental Health Outpatient Co-Responder program is currently seeking a real leader, someone who has the passion, commitment, talent, and ability to work collaboratively with their peers to continue to advance the mission and vision of the organization.

The selected applicant, case manager, will focus on post-crisis outreach, coordination of services, and engagement with community resources, while also supporting documentation and program accountability. Covering both Colquitt and Mitchell Counties will allow this role to operate efficiently while reinforcing the overall effectiveness of the co-responder model and supporting both law enforcement and clinical teams.
