This is a full-time position in one of our Glenwood, IA houses.
The hours are Monday- Wednesday 3pm-10:30pm, and every other Sat. 8am-10:30pm.
Job Title: Supported Community Living Specialist (SCLS)
The SCLS is directly responsible to the Team Leader and the Director of Residential Services.
Specific Duties and Responsibilities:
- Teach, train, and assist individuals in developing the skills necessary for independence in daily living activities, including:
- Coordinating health and medical needs.
- Training individuals to manage their own health and medical needs.
- Developing skills for better community participation.
- Assisting individuals in accessing community services.
- Facilitating participation in community activities each week.
- Assisting in advocacy procedures for rights and needs.
- Preparing individualized diet plans as needed.
- Writing Individualized Intervention Plans.
- Providing emergency/safety skills training.
- Training in personal business and property management.
- Training in budgeting.
- Providing transportation to appointments and activities.
- Training in daily living skills.
- Orient new individuals in their home, assess skills, and provide input for progress reports and Person-Centered Plans (PCP).
- Supervise individual goals and document progress.
- Supervise finances and provide budgeting guidance.
- Provide a supportive and therapeutic atmosphere.
- Supervise care and use of personal property.
- Train new staff on duties, schedules, and policies.
- Follow the assigned work schedule.
- Ensure individuals receiving care are never left unsupervised.
- Complete records and documentation as per procedures.
- Attend staff meetings and training sessions.
- Maintain cleanliness, safety, and health standards in the residence.
- Stay informed of programming and administrative updates.
- Maintain confidentiality of all written and verbal information.
- Document behaviors and assist in intervention plans.
- Handle other duties as assigned.
- Monitor and document physician-ordered diets as needed.
- Assist individuals with daily living activities.
- Ensure safety from environmental hazards.
Essential Functions:
- Provide quality care.
- Transport individuals as needed.
- Provide training in daily living skills.
- Maintain housekeeping standards.
- Work with individuals with behavioral problems.
- Provide a supportive atmosphere.
- Administer medication and maintain documentation after training.
- Exercise sound judgment in emergencies.
- Work according to the assigned schedule, with variations for overnight shifts.
- Complete documentation according to agency requirements.
- Work independently and ensure the welfare of individuals served.
- Comply with HIPAA regulations.
- Assist individuals with daily living activities.
- Maintain confidentiality regarding Protected Health Information (PHI).
Qualifications/Requirements:
- Must not be a carrier of serious infectious or communicable diseases.
- Must be 18 years of age.
- High school diploma or GED required.
- Preferred experience in teaching cooking, cleaning, and personal hygiene skills.
- Sound judgment in decision-making.
- Valid Iowa Chauffeur’s license with an acceptable driving record.
- Successful completion of pre-employment physical, TB test, and drug screenings.
- Completion of a 10-hour med-management class.
- Willingness to work with individuals with behavioral disorders.
- Willingness to clean biological fluids/waste.
- Ability to work as part of a team.
- Good verbal and written communication skills.
- Self-motivated.
- No legal convictions for assault, abuse, or bodily injury.
- Participation in agency investigations with respect for confidentiality.
Physical Requirements:
- Ability to travel up to 70 miles.
- Ability to enter and exit residences.
- Ability to walk up and down stairs with hands full.
- Ability to work a full scheduled shift.
- Ability to carry supplies up to 50 pounds.
- Ability to provide physical care to residents.
- Ability to assist individuals in regaining balance or transferring positions.
- Ability to perform routine household tasks.
- Ability to assist individuals in wheelchairs.
- Ability to assist in the shower or bath.
- Ability to carry groceries and perform snow removal as necessary.
