This position is eligible for a hiring bonus.

This is a full-time position in one of our Glenwood, IA houses.

The hours are Monday- Wednesday 3pm-10:30pm, and every other Sat. 8am-10:30pm.

Job Title: Supported Community Living Specialist (SCLS)

The SCLS is directly responsible to the Team Leader and the Director of Residential Services.

Specific Duties and Responsibilities:

  1. Teach, train, and assist individuals in developing the skills necessary for independence in daily living activities, including:
    1. Coordinating health and medical needs.
    2. Training individuals to manage their own health and medical needs.
    3. Developing skills for better community participation.
    4. Assisting individuals in accessing community services.
    5. Facilitating participation in community activities each week.
    6. Assisting in advocacy procedures for rights and needs.
    7. Preparing individualized diet plans as needed.
    8. Writing Individualized Intervention Plans.
    9. Providing emergency/safety skills training.
    10. Training in personal business and property management.
    11. Training in budgeting.
    12. Providing transportation to appointments and activities.
    13. Training in daily living skills.
  2. Orient new individuals in their home, assess skills, and provide input for progress reports and Person-Centered Plans (PCP).
  3. Supervise individual goals and document progress.
  4. Supervise finances and provide budgeting guidance.
  5. Provide a supportive and therapeutic atmosphere.
  6. Supervise care and use of personal property.
  7. Train new staff on duties, schedules, and policies.
  8. Follow the assigned work schedule.
  9. Ensure individuals receiving care are never left unsupervised.
  10. Complete records and documentation as per procedures.
  11. Attend staff meetings and training sessions.
  12. Maintain cleanliness, safety, and health standards in the residence.
  13. Stay informed of programming and administrative updates.
  14. Maintain confidentiality of all written and verbal information.
  15. Document behaviors and assist in intervention plans.
  16. Handle other duties as assigned.
  17. Monitor and document physician-ordered diets as needed.
  18. Assist individuals with daily living activities.
  19. Ensure safety from environmental hazards.

Essential Functions:

  1. Provide quality care.
  2. Transport individuals as needed.
  3. Provide training in daily living skills.
  4. Maintain housekeeping standards.
  5. Work with individuals with behavioral problems.
  6. Provide a supportive atmosphere.
  7. Administer medication and maintain documentation after training.
  8. Exercise sound judgment in emergencies.
  9. Work according to the assigned schedule, with variations for overnight shifts.
  10. Complete documentation according to agency requirements.
  11. Work independently and ensure the welfare of individuals served.
  12. Comply with HIPAA regulations.
  13. Assist individuals with daily living activities.
  14. Maintain confidentiality regarding Protected Health Information (PHI).

Qualifications/Requirements:

  1. Must not be a carrier of serious infectious or communicable diseases.
  2. Must be 18 years of age.
  3. High school diploma or GED required.
  4. Preferred experience in teaching cooking, cleaning, and personal hygiene skills.
  5. Sound judgment in decision-making.
  6. Valid Iowa Chauffeur’s license with an acceptable driving record.
  7. Successful completion of pre-employment physical, TB test, and drug screenings.
  8. Completion of a 10-hour med-management class.
  9. Willingness to work with individuals with behavioral disorders.
  10. Willingness to clean biological fluids/waste.
  11. Ability to work as part of a team.
  12. Good verbal and written communication skills.
  13. Self-motivated.
  14. No legal convictions for assault, abuse, or bodily injury.
  15. Participation in agency investigations with respect for confidentiality.

Physical Requirements:

  1. Ability to travel up to 70 miles.
  2. Ability to enter and exit residences.
  3. Ability to walk up and down stairs with hands full.
  4. Ability to work a full scheduled shift.
  5. Ability to carry supplies up to 50 pounds.
  6. Ability to provide physical care to residents.
  7. Ability to assist individuals in regaining balance or transferring positions.
  8. Ability to perform routine household tasks.
  9. Ability to assist individuals in wheelchairs.
  10. Ability to assist in the shower or bath.
  11. Ability to carry groceries and perform snow removal as necessary.
