Job summary

Genesiscareers is seeking a Social Services Specialist for Pine Point Center, a 60-bed skilled nursing facility. You will identify social, emotional, and mental health needs and arrange services to enhance quality of life for patients/residents.

You will collaborate with an interdisciplinary team, develop care plans, and conduct meetings with families to support discharge planning and referrals to community resources. NASW Code of Ethics guides our care.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in developing an interdisciplinary care plan for social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information.
• Perform discharge planning, education, documentation, and referrals to appropriate agencies.
• Establish relationships with community-based agencies to benefit patients/residents.
