Job summary

CrescentCare in Louisiana is seeking a qualified social services professional to provide assessment, referral, education, service planning, and care coordination to clients.

The position requires a BA/BS and at least three years of social services experience. A valid driver’s license and reliable transportation are needed. Licensure is not required, and you will work with an interdisciplinary team while pursuing ongoing professional development.

Responsibilities

• Ensures expedient client access to needed resources (food, housing, health insurance, medical & psych care, financial resources).

• Complete psychosocial assessments and create individualized service plans.
• Maintain caseload management and submit notes within deadlines.
• Coordinate with case managers, healthcare providers, psychiatrists, and health educators; participate in huddles and care team meetings.

Works as a part of an interdisciplinary team with CrescentCare and/or community providers. Maintains a close working relationship with direct supervisor, participates in active supervision and remains receptive to feedback on professional development to support high quality service provision.Key Duties and ResponsibilitiesEnsures expedient client access to needed resources (e.g. food, housing, health insurance, medical & psych care, financial resources, etc.)Engage clients in face to face contact approximately 4-5 hours a day; provide a minimum of 24 total documented UOS per day.Complete a psychosocial assessment and create an individualized plan for services with each client.Works according to standard with the assigned care team, if applicable.Takes responsibility for the management of assigned caseload using evidence-based interventions.Submits well-written notes based on treatment/care/recovery plans, thorough strengths-based assessments, and other required paperwork within required deadlines.Develop and maintain strong working relationships with and routinely consult with case managers, healthcare providers, psychiatrists, and health educators both within the agency and the community at large; participate in huddles and care team meetings as requested and able.Responsible for ensuring clarity on program expectations and standards, including but not limited to record keeping and documentation, behavioral expectations, work quality and productivity expectations, effective teamwork, and client-centered ethical practice.Responsible for remaining receptive to coaching and/or participating in disciplinary action when expectations and standards are not met.Attends all meetings and trainings as directed by supervisor.Attends a minimum of 20 hours of continuing education annually.Other duties as assigned by supervisors.QualificationsThe minimum education level required for the position is a BA/BS.The minimum experiential level required for the position is three years providing social servicesProfessional licensure is not required.Minimum age of 21.Must have reliable transportation anda valid driver’s license, no license denials, revocations, or more than one suspension in the past three yearsvalid car insuranceno more than one at-fault accident in the past three yearsno more than two moving violations in the past five yearsTotal Compensation PackageWe believe the investment we make in our employees is among the most important.
