Job summary

CrescentCare in Houma, LA is seeking a qualified Case Manager to provide assessment, referral, education, service planning, and care coordination for clients.

You will work with an interdisciplinary team, maintain documentation, complete psychosocial assessments, and support clients in self-management and adherence to treatment plans; requires BA/BS and 3 years social services experience, and a valid driver’s license.

Qualifications

• The position requires a BA/BS degree and three years of experience providing social services.
• Must have reliable transportation and a valid driver’s license; no licensing issues in past three years.

Responsibilities

• Provide psychosocial assessment and create individualized service plans.
• Coordinate with care team and community providers to connect clients with resources.
• Document treatment plans and progress notes in a timely, compliant manner.
• Engage clients in face-to-face contact and document a minimum number of contacts per day.
• Maintain continuing education and participate in supervision and trainings.

Position Summary

Provides assessment, referral, education, service planning, and care coordination services to CC clients with a focus on increasing self-management and adherence to medical and behavioral health treatment plans. Works as a part of an interdisciplinary team with CrescentCare and/or community providers. Maintains a close working relationship with direct supervisor, participates in active supervision and remains receptive to feedback on professional development to support high quality service provision.

Key Duties and Responsibilities

• Ensures expedient client access to needed resources (e.g. food, housing, health insurance, medical & psych care, financial resources, etc.)
• Engage clients in face to face contact approximately 4-5 hours a day; provide a minimum of 24 total documented UOS per day.
• Complete a psychosocial assessment and create an individualized plan for services with each client.
• Works according to standard with the assigned care team, if applicable.
• Takes responsibility for the management of assigned caseload using evidence-based interventions.
• Submits well-written notes based on treatment/care/recovery plans, thorough strengths-based assessments, and other required paperwork within required deadlines.
• Develop and maintain strong working relationships with and routinely consult with case managers, healthcare providers, psychiatrists, and health educators both within the agency and the community at large; participate in huddles and care team meetings as requested and able.
• Responsible for ensuring clarity on program expectations and standards, including but not limited to record keeping and documentation, behavioral expectations, work quality and productivity expectations, effective teamwork, and client-centered ethical practice.
• Responsible for remaining receptive to coaching and/or participating in disciplinary action when expectations and standards are not met.
• Attends all meetings and trainings as directed by supervisor.
• Attends a minimum of 20 hours of continuing education annually.
• Other duties as assigned by supervisors.

Qualifications

• The minimum education level required for the position is a BA/BS.
• The minimum experiential level required for the position is three years providing social services
• Professional licensure is not required.
• Minimum age of 21.
• Must have reliable transportation and
• a valid driver’s license, no license denials, revocations, or more than one suspension in the past three years
• valid car insurance
• no more than one at-fault accident in the past three years
• no more than two moving violations in the past five years
