Mississippi Department of Employment Security in Long Beach, MS is seeking a Social Services Program Coordinator III to manage and coordinate services for individuals enrolled in the Home and Community-Based ID/IDD Waiver Program.
You will maintain person-centered Plans of Services, update them annually or as circumstances change, and conduct monthly contacts, quarterly reviews, and annual assessments, with travel across the six southern counties.
This Position is for a Social Services Program Coordinator III with an agency location in Long Beach MS.
Serve as the Support Coordinator (SC) for people enrolled in the Home and Community-Based ID/IDD Waiver Program. Manages and maintains caseload of people receiving ID/IDD Waiver Services. Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload. Revises/updates each person's PSS at least annually or when changes in the person's circumstances occur or when requests are made by the person/legal representative. Coordinates and monitors identified supports and services through regular monthly contacts, face-to-face quarterly reviews, and annual assessments and re-certificatioin meetings. Ensures services are adequate, appropriate and meet the person's needs. Travel throughout the six southern counties will be required.
Typically requires a Bachelor's Degree in a related field and 4-7 years experience and one (1) year of experience working with people with IDD. Must be eligible to drive a Satate of MS vehicle.
