Trilogy Health Services in Ohio's Sylvania area is seeking a Full-Time Discharge Planner to support the Social Services Department. You will direct, plan, and develop the operation of social services and participate in discharge planning and resident assessments.
Qualified candidates hold a Bachelor’s in social work with LSW certification and 3–5 years in senior living or long‑term care. Proficiency with Microsoft Office is required, and travel is extensive.
This is a Full-Time Discharge Planner role providing additional support for the Social Services Dept.
Directs, plans, organizes and develops the operation of the Social Services Department. Participates in the implementation of social care plans and resident assessments in accordance with current federal, state and local standards, guidelines and regulations. Ensures that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
