Job summary

Albermarle Health & Rehabilitation Center in Charlottesville, Virginia is seeking a Discharge Planning Assistant to join our caring team. This full-time position offers Monday through Friday schedules and an opportunity to provide discharge planning as well as comprehensive medically related social services to our patients and families.

Responsibilities include MDS, Care Plans, Assessments and timely documentation, all in alignment with state and federal regulations.

Qualifications

• 4-year degree in Social Work, Psychology, Sociology, Counseling, Criminal Justice, or other human services related degree.
• Experience in discharge planning and social services preferred.
• Ability to document within state and federal regulations.

Responsibilities

• Provide discharge planning and social services to patients and families.
• Complete MDS, care plans, assessments and timely documentation.
• Ensure compliance with state and federal regulations in all processes.

Center provides a high volume of short-term nursing and rehabilitation services. We are seeking a candidate that can provide discharge planning as well as comprehensive medically related social services to our patients. Responsibilities include MDS, Care Plans, Assessments and timely documentation operating within the framework for the company's standards for best practice in accordance with all state and federal regulations.

The qualified candidates will have a 4-year degree in Social Work, Psychology, Sociology, Counseling, Criminal Justice, or other human services related degree.
