Qualifications

• Bachelor's degree in social work or human services required.
• State-required certifications/licensures to practice in long-term care.

Responsibilities

• Direct the Social Services department to maximize patients' quality of life and dignity.
• Collaborate with social services staff, interdisciplinary team, and administration.
• Ensure a comprehensive psychosocial assessment is completed for each patient.
• Educate patients and families on rights, decisions, and community resources.
• Provide therapeutic interventions to support adjustment to a nursing center.
