Job summary

Arc at Bradley in Bradley, IL is seeking a Social Services Director to oversee the Social Services department ensuring residents' social, emotional and psychosocial needs are met in accordance with laws and regulations.

You will assess resident needs, develop care plans, collaborate with medical staff and community partners, and lead a team to deliver compassionate, compliant services in a long-term care setting.

Qualifications

• Two years of job-related experience, preferably in long-term care.
• Bachelor’s degree in social work or related field preferred.
• Experience with social work, preferably working directly with patients and their family members at a nursing facility.
• Detail oriented, computer literate and possesses excellent communication skills.

Responsibilities

• Manage and direct the Social Services Department, and identify and provide for residents social, emotional, and psychological needs.
• Accurately assess, document and provide interventions to meet psychosocial needs of all residents.
• Develop care plans based on assessments.
• Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
