Responsibilities

• Assist with resident assessments and documentation related to social services and care planning

• Connect residents with appropriate community resources and services
• Advocate for residents’ needs, preferences, rights, and quality of life

• Provide emotional support and assistance to residents as they adjust to changes in their health and circumstances
• Maintain accurate and timely documentation in accordance with facility and regulatory requirements

We’re looking for a compassionate and dedicated Social Services Director to join our team. While long-term care experience is preferred, we believe a genuine passion for serving seniors and supporting their quality of life matters most.

In this role, you’ll work closely with residents, their loved ones, our care team, and community resources to provide person-centered support. You’ll help residents and families navigate their needs while ensuring each resident’s voice, preferences, and well-being remain at the center of their care.
