The facility seeks a compassionate Social Services Director to lead resident-centered social services within a long-term care setting. You will collaborate with residents, families, care teams, and community resources to support choices and well-being.
Responsibilities include care planning, discharge coordination, advocacy, and documentation while fostering a respectful, person-centered environment for seniors.
We’re looking for a compassionate and dedicated Social Services Director to join our team. While long-term care experience is preferred, we believe a genuine passion for serving seniors and supporting their quality of life matters most.
In this role, you’ll work closely with residents, their loved ones, our care team, and community resources to provide person-centered support. You’ll help residents and families navigate their needs while ensuring each resident’s voice, preferences, and well-being remain at the center of their care.
Assist with resident assessments and documentation related to social services and care planning
Participate in developing and updating individualized care plans
Provide support with behavior management and help identify appropriate interventions
Assist residents and families with discharge planning and transition needs
Build positive relationships with residents, families, and responsible parties
Connect residents with appropriate community resources and services
Advocate for residents’ needs, preferences, rights, and quality of life
Participate in care plan meetings and collaborate with the interdisciplinary care team
Provide emotional support and assistance to residents as they adjust to changes in their health and circumstances
Maintain accurate and timely documentation in accordance with facility and regulatory requirements
Help promote a positive, respectful, and person-centered environment
Social Services degree required.
Long-term care experience preferred.
Passion for working with and serving the senior population.
Strong communication, organization, and interpersonal skills.
Ability to work collaboratively with residents, families, staff, and community resources.
