Job summary

Sprenger Health Care in Bluffton, SC is seeking a dedicated social worker or experienced professional to support residents and families in our long-term care settings. You will develop strong one-to-one relationships, coordinate with hospice agencies, and act as a liaison with social, health and community services.

You will also collaborate with the Activity Department to meet residents' social needs and assist with admissions and discharges.

Qualifications

• 2 years of experience in long term care or related field.
• Licensed Social Worker or bachelor's degree preferred.
• Must possess excellent communication and organizational skills.

Responsibilities

• Develop one-to-one relationships with residents and families.
• Establish relationships with area hospice agencies.
• Act as a liaison with social, health and community agencies.
• Coordinate with Activity Department to meet socialization needs of the residents.
• Oversee coordination of referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology).

Qualifications:

2 years of experience in long term care, licensed social worker or bachelor's degree preferred, must possess excellent communication and organizational skills.
