Job summary

Oak Park Care Center is seeking a compassionate Social Services Director to lead discharge planning and psychosocial support for residents in our senior care setting. This role requires coordinating within the hospital system and ensuring high-quality resident care.

Qualifications

• Must hold a current State Clinical Social Work Licensure.

Responsibilities

• Coordinate social services/discharge planning within the hospital system through effective communication at all levels.
• Develop measurable, realistic plans and objectives for social service daily operations.
• Perform psychosocial assessment and reassessment for residents and families.
• Ensure resident safety and reduce health care errors.
• Formulate teaching plans based on identified learning needs, including families.
• Promote residents’ rights and interpret the Residents’ Bill of Rights.
• Interact professionally with residents and families to shape the plan of care.
• Provide individual and group support during adjustment, crises, or needs.
• Direct Social Service/Discharge Planning to meet resident needs and center policies.
• Provide resources and support to Social Service/Discharge Planning staff.
• Maintain good rapport with medical staff and departments.
• Operate equipment safely and correctly.
• Treat patients and families with respect and dignity.
• Support a culture of safety and quality.

Essential Functions of Social Services Director:

• Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels

• Develop specific, measurable, realistic plans and objectives, which enhance social service daily

• Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their families

• Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors

• Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning.Include family in teaching as appropriate.

• Demonstrate knowledge of clinical social work and discharge planning.Interpret and promote residents’ rights and the "Residents’ Bill of Rights"

• Interact professionally with resident and family and involve resident and family in the formation of the plan of care.

• Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.

• Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and center policy is followed.

• Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.

• Be flexible, organized, and function under stressful situations.

• Maintain a good rapport and working relationship with medical staff, departments, and employees.

• Manage and operate equipment safely and correctly.

• Treat patients and their families with respect and dignity.

• Support and maintain a culture of safety and quality.
