Job summary

Oak Park Care Center is seeking a Social Services Director to lead discharge planning and psychosocial support for residents. You will coordinate within the hospital system, develop measurable plans, assess residents, and advocate for residents' rights while collaborating with families and staff.

This senior role requires empathy, confidentiality, and strong communication in a fast-paced elder care setting.

Qualifications

• High school diploma or GED and current State Clinical Social Work Licensure.
• Experience in social work or long term care environment.
• Ability to read, write and speak English.
• Ability to work cooperatively as a team member.
• Ability to communicate effectively with residents and families.
• Ability to react decisively in emergencies.
• Maintain confidentiality and uphold ethical standards.
• Knowledge of training techniques for clinical staff.

Responsibilities

• Ensure social service/discharge planning is integrated within the hospital system through effective communication.
• Develop measurable plans and objectives to improve social service daily.
• Perform psychosocial assessment and reassessment for all residents.
• Provide resident and family support during adjustment, crisis, or need.
• Coordinate Social Service/Discharge Planning Department to meet resident needs and policy.
• Support Social Service/Discharge Planning staff and clinical staff.
• Maintain rapport with medical staff and departments to ensure safety and quality.

Essential Functions of Social Services Director

• Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels

• Develop specific, measurable, realistic plans and objectives, which enhance social service daily

• Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their families

• Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors

• Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning.Include family in teaching as appropriate.

• Demonstrate knowledge of clinical social work and discharge planning.Interpret and promote residents' rights and the "Residents' Bill of Rights"

• Interact professionally with resident and family and involve resident and family in the formation of the plan of care.

• Provide individual and group help for residents' and families at times of adjustment, crisis, or need.

• Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and center policy is followed.

• Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.

• Be flexible, organized, and function under stressful situations.

• Maintain a good rapport and working relationship with medical staff, departments, and employees.

• Manage and operate equipment safely and correctly.

• Treat patients and their families with respect and dignity.

• Support and maintain a culture of safety and quality.

Qualifications of Social Services Director

• A high school diploma/GED and current State Clinical Social Work Licensure are required

• Experience in social work or long term care environment is required

• Candidates must maintain a working email address and phone number for employer communication.

• Ability to read, write and speak the English language

• Ability to work cooperatively as a member of the team

• Ability to communicate effectively with residents and their family members, and at all levels of the organization

• Ability to react decisively and quickly in emergency situations
