Job summary

Steelville Care and Rehab seeks a Social Services Director to coordinate discharge planning, assess psychosocial needs, and lead the social services team in providing compassionate, compliant care.

You will ensure safety, rights awareness, and family involvement, while partnering with clinical staff to deliver high-quality resident support.

Qualifications

• Bachelor’s degree in social work or related human services field (sociology, special education, rehabilitation counseling, psychology).
• Preferred: one year of supervised social work experience in healthcare.
• Current State Clinical Social Work Licensure.
• Ability to work cooperatively as a team member.
• Ability to communicate effectively with residents and families at all levels.

Responsibilities

• Coordinate social service/discharge planning within the hospital system through effective communication.
• Develop specific, measurable objectives to enhance daily social service operations.
• Perform psychosocial assessments and reassessments for all residents and address needs of residents and families.
• Ensure resident safety and reduce medical/health care errors.
• Create teaching plans based on learning needs and involve family in teaching.
• Promote Residents’ Bill of Rights and residents’ rights.
• Interact professionally with residents and families and include them in care plans.
• Provide individual and group support during adjustment, crisis, or need.
• Direct the Social Service/Discharge Planning Department to meet resident needs and comply with policy.
• Provide resources and support to Social Service/Discharge Planning staff and clinical staff.
• Maintain a culture of safety and quality and collaborate with medical staff.

Essential Functions of Social Services Director:

• Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels
• Develop specific, measurable, realistic plans and objectives, which enhance social service daily
• Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their family
• Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors
• Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning.Include family in teaching as appropriate.
• Demonstrate knowledge of clinical social work and discharge planning.Interpret and promote residents’ rights and the “Residents’ Bill of Rights”
• Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
• Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
• Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and facility policy is followed.
• Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
• Be flexible, organized, and function under stressful situations.
• Maintain a good rapport and working relationship with medical staff, departments, and employees.
• Manage and operate equipment safely and correctly.
• Treat patients and their families with respect and dignity.
• Support and maintain a culture of safety and quality.

Qualifications of Social Services Director:

• Bachelor’s degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology
• One year of supervised social work experience in a health care setting working directly with individuals is preferred
• Current State Clinical Social Work Licensure
• Candidates must maintain a working email address and phone number for employer communication.
