Job summary

The Lodge at Sandhills Carolina, a SanStone Health & Rehabilitation facility in Pinehurst, NC, seeks an experienced Social Services Director to join our team. You will collaborate with caregivers to provide high-quality patient care and oversee care planning, referrals, and documentation to ensure excellent resident outcomes.

Qualifications

• Previous experience in social work, with a focus on short term and long-term care patients.

Responsibilities

• Assess nature of patients' situations by interviewing and reviewing personal history.
• Develop and execute individual treatment plans/care plans.
• Determine appropriate milestones to gauge patient progress.
• Communicate patient updates to all relevant parties.
• Facilitate referrals to other healthcare professionals and programs as needed.
• Maintain accurate documentation.
