Job summary

The Lodge at Sandhills Carolina in Pinehurst, NC, is seeking an experienced Social Services Director to join our team. You will collaborate with caregivers to provide high-quality patient care and lead social services initiatives on a new, state-of-the-art campus.

Responsibilities include assessing patient needs, developing care plans, coordinating referrals, and maintaining documentation, while fostering a compassionate, collaborative work environment.

• Experience in social work with short- and long-term care focus.
• Strong leadership and people management abilities.
• Excellent written and verbal communication skills.

Responsibilities include:

• Assess nature of patients' situations by interviewing and reviewing personal history.
• Develop and execute individual treatment plans/care plans.
• Determine appropriate milestones to gauge patient progress.
• Communicate patient updates to all relevant parties.
• Facilitate referrals to other healthcare professionals and programs as needed.
• Maintain accurate documentation.

• Previous experience in social work, with a focus on short term and long-term care patients
