Job summary

Sprenger Health Care Bluffton seeks a dedicated Social Services professional for our long-term care community. You will develop one-to-one relationships with residents and families, establish connections with area hospice agencies, and act as a liaison with social, health and community services.

You will coordinate with the Activity Department to meet socialization needs, oversee referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology), and assist

Qualifications

• 2 years of experience in long term care.
• Excellent communication and organizational skills required.
• Licensed social worker or bachelor’s degree preferred.

Responsibilities
