Rehab at Scottsdale Village Square is seeking a compassionate and experienced Social Services Director to lead our Social Services department and ensure residents' psychosocial and emotional needs are met.

The role includes overseeing staff, developing care plans with an interdisciplinary team, coordinating admissions and discharges, and advocating for residents' rights while ensuring compliance with federal and state regulations.

Competenze

• Bachelor's degree required; master's degree preferred.
• Minimum of two years of supervised social work experience in a healthcare setting, preferably long-term care or skilled nursing.
• Knowledge of federal and state regulations governing skilled nursing facilities.
• Strong communication, documentation, and organizational skills.

Role Summary

Rehab at Scottsdale Village Square is seeking a compassionate and experienced Social Services Director to lead our Social Services department. The Social Services Director plans, organizes, develops, and directs the overall operation of the Social Services department in accordance with current federal, state, and local standards, guidelines, and regulations, and in accordance with our established policies and procedures, to ensure residents' psychosocial and emotional needs are met.

Responsibilities

• Oversee the day-to-day operations of the Social Services department and supervise social services staff
• Assess residents' psychosocial, emotional, and social needs upon admission and throughout their stay
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
