Job summary

The Blossoms in Rogers, AR is looking for a Social Services Director to lead resident-centered care planning and coordinate interdisciplinary teams. This role focuses on ensuring high-quality social work support for residents and their families.

You will develop individualized care plans, guide admission and discharge processes, and oversee staff to maintain compassionate, compliant social services. This position emphasizes collaboration and growth within our care-focused facility.

Qualifications

• Bachelor's degree in social work or related field.
• At least 1 year of social work experience.
• Experience in long-term care or sub-acute setting preferred.

Responsibilities

• Maintaining and/or improving residents' ability to control everyday physical needs and psychosocial needs.
• Identifying individual social and emotional needs.
• Developing and maintaining individualized care plans.
• Participating in admission planning, community re-entry, or transfer discussions.
• Participating in interdisciplinary care plan meetings to address social and emotional needs.
• Collaborating with the finance team to educate residents and ensure proper documents during stay.
• Assisting with discharge planning and arranging DME/home care for safe community re-entry.

Key Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Skills, Knowledge and Expertise

• Bachelors degree Social field is required
• At least one year experience in Social Work field required
• Experience in long term care and/or sub-acute setting preferred
