Job summary

Harborview Health Systems in Sandy Springs, GA is seeking a compassionate Social Services Assistant for our nursing home.

You will partner with licensed social workers, residents, and families to plan and implement care within federal and state guidelines while promoting dignity and person-centered care.

Responsibilities

• Assist in planning, organizing, implementing, and evaluating the facility’s Social Services Department under the direction of a Licensed Social Worker
• Ensure social services are delivered in compliance with federal, state, and local guidelines and Harborview's values
• Help develop and monitor individualized care plans for residents, addressing their social and emotional needs
• Provide ongoing support to residents and families during admissions, transitions, and care conferences
• Recommend equipment, services, or referrals for outpatient and home health support
• Organize and participate in in-service training sessions for the social services team and other staff
• Advocate for residents and serve as a liaison among staff, families, and external agencies
• Collaborate with interdisciplinary care teams to ensure holistic, person-centered care

What You Will Do
