Job summary

Confidential Skilled Nursing Facility in Delaware County, PA is seeking a Social Services Director to oversee social work services for residents. This role focuses on developing comprehensive care plans, coordinating discharge planning, and supporting patients and families to enhance quality of life.

The ideal candidate holds a Bachelor’s degree in Social Work or related field and has experience in healthcare social work. Strong communication and interagency collaboration are essential.

Qualifications

• Bachelor's degree from an accredited school of Social Work or related field is required
• At least one year of supervised social work experience in a healthcare setting, working directly with individuals, is preferred
