Job summary

Willow Brook Rehabilitation and Healthcare Center in Kutztown, Pennsylvania is seeking a compassionate Social Services Director to lead our interdisciplinary team and advocate for residents' rights and well-being. You will develop individualized social services plans, coordinate with families, perform discharge planning, and build partnerships with community agencies to support quality of life.

A Bachelor's degree in Social Work is required, with at least one year of supervised experience

Qualifications

• Bachelor's degree from an accredited school of Social Work or related field.
• At least one year of supervised social work experience in a healthcare setting, working directly with individuals, is preferred.

Responsibilities

• Develop and implement comprehensive care plans for each resident's social services needs.
• Hold meetings with patients/residents, families, and significant others to provide support and improve quality of life.
• Perform patient/resident discharge planning activities, including education, documentation, and referrals to appropriate agencies.
• Establish relationships and maintain contact and referral flow with community-based agencies and services.
