Job summary

Avalon Health & Rehabilitation Center in Pasco, WA is seeking a Social Services Director. This full-time position leads the facility's social services program, advocating for resident well‑being, conducting psychosocial assessments, and supporting individualized care planning and transitions of care.

You will collaborate with the interdisciplinary team to promote positive resident outcomes, ensure regulatory documentation, and connect residents with community resources.

Qualifications

• Bachelor's degree in Social Work preferred.
• Two years of social work experience required.
• Healthcare or skilled nursing experience preferred; knowledge of state and federal regulations.
• Strong communication, assessment, and documentation skills.
• Basic computer proficiency and ability to pass employment screenings.

Responsibilities

• Lead the facility's social services program and advocate for resident well‑being

• Assist residents and families with adjustment, discharge planning, and transitions of care
• Connect residents and families with community resources and support services
