Job Summary:

Key Responsibilities:

• Leadership and Management: Oversee the Social Services Department, fostering a collaborative environment among team members to ensure excellent patient care and satisfaction.


• Case Management: Develop and implement individualized care plans, ensuring that each resident receives personalized attention and support to achieve their rehabilitation goals.


• Communication: Coordinate with multidisciplinary teams, including physicians, therapists, and nurses, to ensure seamless communication and effective patient care. This includes conducting timely 72-hour meetings, care plan meetings, and discharge planning.


• Staff Development: Provide guidance, training, and mentorship to the Social Worker, promoting professional growth and development.


• Compliance and Quality Improvement: Ensure compliance with regulatory requirements and industry standards, while continuously identifying and implementing quality improvement initiatives. Must have familarity with PASSARs, LOCs, and updating Ombudsman Discharge Portal. Must meet timely completion of MDS assesments.
