Job summary

AuthentiCare Health Centers in Rhode Island is seeking a Social Services Director to lead resident-centered care across long-term, sub-acute, and memory care services. You will oversee discharge planning, care plans, and family support while coordinating with the interdisciplinary team.

Ideal candidates hold a BSW or related field and are a licensed Social Worker. Geriatric or long-term care experience, strong creativity and organizational skills, and excellent verbal and written communication

Qualifications

• BSW or related fields are required.
• Prior work with a geriatric population is a plus.
• Licensed Social Worker in the Commonwealth of MA.
• Two years of Social Work experience in long-term care or healthcare facility preferred but not required.
• Previous management experience is a plus.
• Creativity, organizational skills, and a cheerful personality.
• Experience in long-term care or healthcare settings is a plus.
• Experience with Alzheimer's/Dementia patients is a plus.
• Regular attendance is required.
• Good verbal and written communication skills.
• Confidentiality regarding employee, resident and company information.
• Ability to relate professionally with patients, families, and staff.
• Computer skills sufficient to perform the job.
• Knowledge of HIPAA and state/federal regulations.
• Excellent interpersonal skills for communication with patients and teams.
• Strong organizational skills to manage multiple tasks.

Responsibilities

• Discharge planning, behavior, and care plan meetings.
• Assist residents and families with coping with environment and capacity changes, end-of-life concerns, and grieving.
• Outline goals at admission with patient, family, and team; create a plan to meet them.
• Assess residents' psychological status during admissions and intervals.
• Assess residents' psychological status during Admission.
• Knowledgeable in PASSRS, MDS, Annuals, and Quarterlies during residency.
• Collaborate with healthcare team to deliver comprehensive resident care.
• All other duties as assigned.

RESPONSIBILITIES

Including, but not limited to:

• Conduct discharge planning, behavior, and care plan meetings.
• Assist residents and families with coping with the new environment and changes to physical or mental capacity, end-of-life concerns, and the grieving process.
• Work directly with the patient, family, and other team members to outline the goals at admission and create a plan to meet those goals.
• Responsible for assessing our resident's psychological status during admissions and other intervals during residence.
• Assessment of our residents' psychological status during Admission.
• Knowledgeable in PASSRS, MDS, Annuals, and Quarterlies during their residency.
• Collaborate with healthcare team members to deliver comprehensive resident care in our facilities.
• All other duties as assigned.

SKILLS

• BSW or related fields are required.

• Must be able to maintain confidentiality regarding employee, resident and company proprietary information.
• Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels.
• Must possess computer skills of the type and at a level necessary to accomplish the job.
• Knowledge of HIPPA, and state and federal regulations.
• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
• Strong organizational skills to manage multiple tasks efficiently.
