Job summary

AuthentiCare Health Centers seeks a Director of Nursing to lead nursing services across long-term, sub-acute, and memory care populations. The DON will supervise licensed and non-licensed staff, ensure regulatory compliance, and partner with interdisciplinary teams to deliver resident-centered care.

The ideal candidate has an ADN or higher, 2–4 years of leadership in long-term care, and strong knowledge of MDS, regulations, and PCC EMR. Competitive benefits and a $5,000 sign-on bonus are offered.

Qualifications

• Associate's degree in nursing required; bachelor's degree preferred.
• 2–4 years of nurse leadership experience in long-term care.
• Knowledge of state and federal regulations.
• Experience with geriatric psychiatric population.

Responsibilities

• Plans, directs and evaluates nursing care to meet clinical and financial outcomes.
• Provides guidance and supervision to non-professional staff.
• Ensures care assignments match staff competencies and patient needs.
• Schedules and monitors nursing staff; reports results to leadership.
• Promotes a positive work environment and supports patients and families.
• Updates patients and care teams on questions and requests.
• Ensures reporting to Executive Director/Administrator.
• Communicates significant changes in residents' conditions to physicians and families.

RESPONSIBILITIES

Including, but not limited to:

• Plans, organizes, directs, supervises and evaluates nursing care provided to achieve clinical and financial outcomes.

• Provides professional guidance and supervision to non-professional staff.

• Ensures patient care assignments are consistent with staff competencies, scope of practice and the needs of the patients.

• Completes patient care requirements by scheduling and assigning nursing and staff; follows up on work results

• Establishes a positive work environment by providing support to patients and families

• Provides information to patients and health care team by answering questions and requests

• Ensures complete and prompt reporting to the Executive Director / Administrator

• Ensures that significant changes in residents' conditions are communicated to the physician and family or responsible party

• Associate's degree in nursing required; bachelor's degree in nursing preferred

• 2-4 years of experience in nurse leadership in a long-term care setting

• Knowledge of state and federal regulations

• Experience with geriatric psychiatric population

• Assess resident health problems and needs, develop and implement nursing care plans.

• Supervises the administration of medications and treatments as prescribed by physicians.

• Monitor resident progress and respond to changes in condition.

• Collaborate with multidisciplinary teams to provide comprehensive care.

• Educate residents and their families regarding health conditions, treatment plans, and discharge.

• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.

• Participate in case management activities to ensure continuity of care.

• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.

SKILLS

• Registered or Licensed Nurse in the state of Massachusetts

• 5 years or more of nursing experience

• Leadership experience

• Medication administration

• EMR: PCC (Point Click Care)

• Proficiency in memory care techniques and protocols.

• Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.

• Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units

• Strong background in case management practices.

• Ability to work effectively in a medical office environment.

• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.

• Strong organizational skills to manage multiple tasks efficiently.
