Position Description

Responsible for identifying psychosocial, mental, and emotional needs, and providing, developing, or aiding in the access of services to meet those needs. Services will be provided in accordance with the NASW Code of Ethics, and in compliance with Federal, State, and Local regulations.

Qualifications

• Certified, licensed, or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a related Human Services field from an accredited school, required.
• One year of supervised social work experience with geriatric individuals in a healthcare setting.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Complete social history and psychosocial assessments for residents, including relevant parts of the MDS, to guide care planning.
• Maintain accurate documentation in compliance with regulations and policies, including assessments, care plans, progress notes, and discharge planning.
• Support residents' rights to self-determination; document refusals and inform residents of alternatives.
• Develop interdisciplinary care plans addressing psychosocial needs and goals.
• Provide therapeutic interventions to help residents cope with their transition and adjustment.
• Support and educate residents and families, referring to external services when necessary.
• Coordinate discharge planning and referrals for home care services.
• Work with the interdisciplinary team to promote resident rights and well-being, addressing abuse as mandated.
• Assist in planning and evaluating social service programs and prepare for inspections.
• Other duties as assigned.
