Job summary

Scenic Wellness Nursing & Rehabilitation in Missouri is seeking a compassionate Social Services Director to lead our Social Services department and advance a person-centered environment that respects dignity, independence, and quality of life for residents.

You will oversee psychosocial assessments, coordinate discharge planning, ensure regulatory compliance, and partner with an interdisciplinary team to address residents’ needs, families’ concerns, and community resources.

Qualifications

• Certified, licensed, or registered Social Worker in Missouri (required).
• Bachelor’s degree in Social Work or Human Services (Sociology, Psychology, etc.) required.
• At least 1 year of supervisory social work experience in a healthcare setting with geriatric populations.
• 2+ years of skilled nursing facility experience preferred.
• Strong leadership, communication, and assessment skills.

Responsibilities

• Oversee Social Services operations per NASW Code and regulations.
• Foster a resident-centered climate supporting psychosocial well‑being.
• Complete psychosocial histories, assessments, and MDS-related care planning.
• Collaborate with the interdisciplinary team to address psychosocial needs.
• Maintain compliant documentation (care plans, progress notes, PASARR, discharge planning).
• Coordinate discharge planning, referrals, and post-care arrangements.
• Educate residents and families on placement, adjustment, and community resources.
• Assist in policy development and annual survey readiness.

Scenic Wellness Nursing & Rehabilitation is seeking a compassionate and dedicated Social Services Director to lead our Social Services department. In this role, you will help create a supportive, person-centered environment that promotes dignity, independence, and quality of life for our residents. If you’re a mission-driven leader who thrives in long-term care and enjoys helping residents and families navigate complex emotional, social, and healthcare needs—this is the role for you.

What You’ll Do

• Oversee and manage all Social Services operations in accordance with NASW Code of Ethics and all Federal, State, and Local regulations.
• Foster a positive, resident-centered climate that supports individuality and psychosocial well‑being.
• Complete psychosocial histories, assessments, and relevant MDS sections; use MDS data to support care planning.
• Participate in care planning and work collaboratively with the interdisciplinary team to address psychosocial needs.
• Monitor and support resident rights, mental health, and adjustment to the facility.
• Maintain accurate, compliant documentation (care plans, progress notes, behavior tracking, PASARR, discharge planning, etc.).
• Educate and support residents and families through placement, adjustment, discharge planning, and access to community resources.
• Coordinate the discharge planning process, referrals, and post-care arrangements.
• Assist in policy development and annual survey readiness.
• Perform additional duties as assigned.

What We’re Looking For

• Certified, licensed, or registered Social Worker in the state of Missouri (required).
• Bachelor’s degree in Social Work or Human Services (Sociology, Psychology, Special Education, Rehab Counseling, etc.) required.
• At least 1 year of supervisory social work experience in a healthcare setting working with geriatric populations.
• 2+ years of skilled nursing facility experience preferred.
• Strong leadership, communication, and assessment skills.
