Job summary

Saber Health Group is seeking a Social Services professional to lead the Social Services Department in a skilled nursing facility, ensuring compassionate, compliant, and person-centered care. You will design, implement, and evaluate social programs to support residents’ emotional and social needs, in line with regulations and facility policies.

The role requires a Bachelor’s in social work (or related field) and at least one year of healthcare social work experience, with strong regulatory

• Bachelor’s degree in social work or related field as required by state/federal rules.
• At least 1 year of supervised social work experience in healthcare settings.
• Knowledge of regulations governing social services in nursing facilities.
• Basic computer skills and ability to navigate electronic health records.

Position Summary:

The primary purpose of your job is to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. You will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Skilled Nursing community’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures. You will ensure that an on-going social service program is designed to meet, in accordance with the comprehensive assessment, psychosocial well-being of each resident.

Education/Qualification:

• Must possess a Bachelor’s degree in social work or in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology, if required by state/federal regulations OR possess a current, unencumbered license to practice as a Social Work within the state in which you work, if required by state/federal regulations OR possess minimum qualifications to work in Social Services as required by the state/federal regulation.

• Must have, as a minimum, one (1) year supervised social work experience in a healthcare setting working directly with individuals, if required by federal and/or state regulations.

• Must be knowledgeable of regulations governing activity services in nursing care facilities.

• Must possess basic computer skills and ability to navigate through various software programs, including electronic health record software.
