Job summary

Crown Care Center in Harrisonville, MO is seeking a Social Services Director to oversee psychosocial care for residents in a long-term care setting, performing assessments, advocacy, and discharge planning. You will coordinate with Nursing and care teams to support resident well-being, handle crisis intervention, mandated reporting, and care coordination for residents with psychosocial risk factors.

Bachelor’s in Social Services and healthcare social work experience required; long-term care

Qualifications

Responsibilities

• Provide assessment, advocacy, and discharge planning for psychosocial needs of patients in a long-term care setting.

We are seeking a full-time Social Services Director / long-term care Social Worker to provide assessment, advocacy, and discharge planning for the psychosocial needs of patients of all ages. This role requires expertise in crisis intervention, mandated reporting, and care coordination for patients with psychosocial risk factors. Responsible for education and advocacy for patients, families, and significant others.

The Social Services Director coordinates the psychosocial needs of residents and families and works closely with the Nursing Department to support resident care in a long-term care / healthcare facility.

Social Services Director Requirements:

• Bachelor’s Degree in Social Services

• Experience in healthcare social work

• Long-term care social work experience a plus
