Job summary

Signature HealthCARE seeks a Social Services Director for weekend, part-time hours at our Bowling Green facility. You will support residents and families with social, emotional, and family needs and contribute to discharge planning and care coordination.

You will ensure timely documentation, participate in care planning, and act as a resource for residents and families while upholding confidentiality and professional standards.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ years of experience in social services, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation to comply with Facility, State, and Federal policies.
• Participate in resident care planning.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

How you Will make a difference

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

What you Need to make a Difference

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.
