Outfield Healthcare Partners is seeking a dedicated Social Services Director (LSW) to lead the Social Services program in a skilled nursing facility. You will supervise the department, ensure compliant psychosocial care, and advocate for residents’ rights and quality of life.

You will oversee discharge planning, care transitions, and interdisciplinary collaboration with nursing, therapy, and admissions teams, while maintaining accurate documentation and ensuring survey readiness.

Qualifications

• Bachelor’s degree in social work or related field.
• Minimum 1 year of supervisory social work experience in healthcare.
• Experience working with geriatric residents.
• Strong discharge planning and psychosocial assessment knowledge.

Responsibilities

• Lead Social Services department in compliance with federal/state guidelines.
• Complete social histories and psychosocial assessments; develop care plans.
• Coordinate discharge planning and community referrals.
• Support residents and families with transition planning and advocacy.
• Participate in interdisciplinary care plan meetings and QA initiatives.
• Assist with Advance Directives and psychosocial needs.
• Collaborate with hospitals, hospice, and outpatient providers.
• Maintain accurate documentation in EMR systems; ensure readiness for state surveys.

What You’ll Do

As the Social Services Director, you will oversee the facility’s Social Services program and ensure residents receive the emotional, psychosocial, and advocacy support they need to thrive.

Primary Responsibilities

• Lead and manage the Social Services department in compliance with federal, state, and company guidelines
• Complete social histories, psychosocial assessments, care plans, and required documentation
• Coordinate discharge planning and community resource referrals
• Support residents and families with transition planning, emotional support, and resident rights advocacy
• Participate in interdisciplinary care plan meetings and quality assurance initiatives
• Assist residents with Advance Directives and psychosocial needs
• Collaborate with hospitals, hospice agencies, outpatient providers, and community organizations
• Maintain accurate documentation within PointClickCare and other systems
• Educate residents, families, and staff on resident rights, abuse prevention, and psychosocial care
• Prepare the department for state survey readiness and compliance

Qualifications

Required

• Licensed Social Worker (LSW) in the State of Indiana
• Bachelor’s degree in social work or human services-related field
• Minimum 1 year of supervisory social work experience in a healthcare setting
• Experience working with geriatric residents
• Strong understanding of discharge planning, care plans, and psychosocial assessments

Preferred

• Skilled Nursing Facility (SNF) or Long-Term Care experience
• Experience with PointClickCare or electronic medical records
• Knowledge of Medicare, Medicaid, PASRR, and state/federal regulations
