Job summary

Outfield Healthcare Partners in Plainfield, Indiana, seeks a dedicated Social Services Director (LSW) to lead the facility's social services program, advancing emotional support, advocacy, discharge planning, and resident rights protection for seniors.

You will collaborate with nursing, therapy, admissions, and leadership to ensure compliant, person-centered care and prepare for state survey readiness.

Qualifications

• Licensed Social Worker (LSW) in Indiana.
• Bachelor’s Degree in Social Work or Human Services-related field.
• Minimum 1 year of supervisory social work experience in a healthcare setting.
• Experience working with geriatric residents.
• Strong understanding of discharge planning, care plans, and psychosocial assessments.

Responsibilities

• Lead and manage the Social Services department in compliance with federal, state, and company guidelines.
• Complete social histories, psychosocial assessments, care plans, and required documentation.
• Coordinate discharge planning and community resource referrals.
• Support residents and families with transition planning, emotional support, and resident rights advocacy.
• Participate in interdisciplinary care plan meetings and quality assurance initiatives.
• Assist residents with Advance Directives and psychosocial needs.
• Collaborate with hospitals, hospice agencies, outpatient providers, and community organizations.
• Maintain accurate documentation within PointClickCare and other systems.
• Educate residents, families, and staff on resident rights, abuse prevention, and psychosocial care.
• Prepare the department for state survey readiness and compliance.
