Job summary

Harmony Chicago is seeking a Social Services Designee to join our compassionate team in Albany Park, Chicago. You will support residents' social and emotional needs, coordinate discharges, and participate in resident care conferences.

We require a high school diploma or equivalent and two years in a Social Worker role, preferably in a healthcare setting; a bachelor’s degree in Social Work or related field is preferred.

Qualifications

• As a minimum, a high school diploma or equivalent and two years of experience in a Social Worker role, preferably in a healthcare setting.
• Prior Social Services Designee experience in a Skilled Nursing Facility is preferred.
• A bachelor’s degree in Social Work or similar field is preferred.

Responsibilities

• Assist in preparing residents for their future and ensure social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences.
• Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
