Job summary

Harmony Chicago is seeking a Social Services Designee to join our skilled nursing team in Albany Park. You will support residents’ social and emotional needs, coordinate discharges, and participate in care conferences to ensure a high standard of care.

The role requires a high school diploma and two years of Social Worker experience (healthcare preferred); a bachelor’s degree in Social Work or related field is preferred.

Qualifications

• As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role preferably in a healthcare setting.
• Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social work or similar field and five (5) years of related experience is preferred.

Responsibilities

• Assist in preparing residents for their future and ensure social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences to maintain continuity of care.
• Support residents with dignity and ensure a high standard of care within a collaborative, inclusive environment.
