Avantara Libertyville is seeking a Social Service Designee to support Guests with emotional and social needs, aligning with federal, state, and local regulations. The role involves collaboration with the care team to enhance residents' quality of life and ensure smooth care transitions.

You will conduct psychosocial assessments, coordinate discharge planning, and provide guidance to Guests and families while maintaining compassionate, person-centered care within a hospital-like skilled nursing

Qualifications

• Two years' experience in a social worker role.
• Bachelor's degree in social work or similar field preferred.
• Healthcare setting experience preferred.

Responsibilities

• Maintain up-to-date evaluation documentation compliant with regulations.
• Develop psychosocial assessments including cultural background and care implications.
• Orient Guests and families to services, rights and facility rules.
• Coordinate discharge planning with Guests, families and staff.
