Job summary

Avantara Mountain View is seeking a Social Services Designee to join our friendly team. You will help residents with their social and emotional needs, coordinate discharges, and participate in care conferences to ensure high quality of life for residents.

Requirements include a high school diploma, 2 years of social work experience in healthcare, and preferred bachelor’s degree in Social Work or related field; five years of related experience preferred but we invest in the right candidate.

Qualifications

• High school diploma or equivalent; 2 years of experience in a Social Worker role in healthcare preferred.
• Bachelor’s degree in Social Work or similar field preferred.
• Five years of related experience is preferable but we are willing to invest in the right candidate.

Responsibilities

• Assist in preparing residents for their future and ensure social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
• Ensure residents receive high standard of care and support their social wellbeing.
