Job summary

Avantara Joliet is seeking a Social Services Designee to join our friendly, caring and supportive team in a Skilled Nursing setting.

You will assist in preparing residents for their futures, coordinate resident discharges, and participate in resident care conferences, ensuring residents’ social and emotional needs are met with high standards of care.

Qualifications

• Prior Social Services Designee experience in a Skilled Nursing Facility.
• Bachelor’s degree in Social work or similar field and three years of related experience preferred.

As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Jolietby ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social work or similar field and three (3) years of related experience is preferable.
