Avantara Joliet is seeking a Social Services Designee to join our compassionate team in Joliet, IL. You will assist residents with social and emotional needs, coordinate discharges, and participate in care conferences to ensure high-quality resident experiences.

Qualifications

• Experience in a Social Services Designee role in a Skilled Nursing Facility.
• Bachelor’s degree in Social Work or a related field.
• Approximately 3 years of related experience preferred.

Responsibilities

• Assist in preparing residents for their future and ensure social needs are met.
• Coordinate resident discharges and participate in care conferences.
• Support residents’ social and emotional well-being during their stay.

As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Joliet by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social work or similar field and three (3) years of related experience is preferable.
