Job summary

Greene Point Health & Rehab is a nonprofit, mission-driven center where you’ll be valued for the vital social work you provide. The role directs the Social Services Program, counsels patients and families on emotional needs, coordinates admissions/discharges, and interacts with community agencies to promote high-quality care.

You will supervise social services staff as needed and contribute to committee work while upholding compassionate, servant leadership.

Qualifications

  • Bachelor degree preferred in social work or related field.
  • Minimum two years supervisory experience in a hospital, skilled nursing center, or medical center.

Responsibilities

  • Plan and direct the Social Services Program.
  • Coordinate admissions and discharges.
  • Counsel patients and families including issues around death or dying.
  • Complete social service portion of the MDS within 14 days of admission.
  • Coordinate with social, health, and community agencies.
  • Facilitate Patient and Family Councils as appropriate.
  • Assist with grievance resolution when requested.

Responds to expressed concerns while displaying a helpful, caring demeanor, answers questions when appropriate in a professional mannerMINIMUM QUALIFICATIONS Minimum of a Bachelor Degree preferred from an approved social work course of study is preferred or minimum of two years’ experience in a supervisory capacity in a hospital, skilled nursing center or other medical center.SUPERVISORY RESPONSIBILITIESSupervision of social services assistant(s), if necessary.LANGUAGE SKILLSAbility to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the organization.MATHEMATICAL SKILLSAbility to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.REASONING ABILITYAbility to apply common sense understanding to carry out instructions furnished in written, oral or diagram format.
