Job summary

Care Initiatives seeks a compassionate Social Services Coordinator to support residents throughout their healthcare journey in our Iowa facilities. You will assess psychosocial needs, document care plans, and collaborate with the care team to improve quality of life.

You’ll provide counseling, assist with discharge planning, and coordinate referrals to community resources. A Bachelor’s in social work or related field plus LS W eligibility are required, with at least 1 year of healthcare social

Qualifications

• Bachelor’s degree in social work or related health/human services field.
• Ability to obtain a Licensed Social Worker (LSW).
• Minimum of one year of supervised social work experience in healthcare.
• Strong communication and empathy when engaging with residents and families.
• Commitment to compassionate, respectful, and discreet care.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories
• Complete assigned MDS sections accurately and on time
• Document care plans, progress, and follow-up per regulations
• Provide counseling and emotional support to residents and families
• Support residents and families with adjustment, rights, and orientation
• Assist with discharge planning and care transitions
• Coordinate referrals and act as a liaison with community resources
• Partner with the care team to enhance quality of life and outcomes
• Plan and support resident, family, and community engagement activities
• Promote facility programs and services to referral partners
