Job summary

Care Initiatives is seeking a compassionate Social Services Coordinator to support residents and families through assessment, counseling, discharge planning, and coordinated care within our skilled nursing communities in Iowa.

Applicants should hold a Bachelor’s in Social Work or related field, be eligible for LSW, and have at least one year of supervised healthcare social work experience. Empathy and strong communication are essential.

Qualifications

• Bachelor's degree in social work, gerontology, or related health & human services field.
• Ability to obtain a Licensed Social Worker (LSW).
• Minimum 1 year of supervised social work experience in a health care setting.
• Strong interpersonal and communication skills for effective interaction.
• Demonstrated empathy, respect, patience, and discretion.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories.
• Complete assigned MDS sections accurately and on time.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support residents and families with adjustment, rights, and orientation.
• Assist with discharge planning and care transitions.
• Coordinate referrals and liaise with community resources.
• Partner with care team to improve quality of life and outcomes.
• Plan and support resident, family, and community engagement activities.
• Promote facility programs and services to referral partners.
