Job summary

United Methodist Communities' Wesley by the Bay in Ocean City, NJ, is seeking a Social Services Coordinator - Housing. This full-time role offers $21.01-$23.63 per hour and supports residents with housing and supportive services, including HUD reporting and transitions in care.

You will connect residents to services, monitor frail residents post-hospital care, maintain resource guides, and produce monthly reports while ensuring HUD compliance and strong community engagement.

Qualifications

• High School Diploma or GED required.
• 3+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting preferred.

Responsibilities

• Connects residents and home-based non-residents to appropriate supportive services and assists with transitions to higher levels of care when needed.
• Monitors the frailest residents through regular contact and post-hospital/therapy transitional care.
• Develops and maintains a resource guide, and produces monthly service reports and annual HUD reporting.
• Creates communication tools (newsletter, website, social media, marketing materials) and serves as liaison to resident groups.
• Coordinates socialization opportunities, including events, classes, spiritual resources, and vocational/educational programming, to reduce isolation.
• Works closely with management to ensure residents meet lease obligations and remain in compliance with HUD requirements.

What You'll Need
