Job summary

Wesley by the Bay in Ocean City, NJ is seeking a Social Services Coordinator - Housing to connect residents with supportive services and manage transitions to higher levels of care. The role is full-time with a schedule of 8:30am - 5:00pm and a pay range of $21.01 - $23.63 per hour based on experience.

Qualifications

• High School Diploma or GED required.
• 3+ years of experience in services for the elderly or social services in healthcare.
• Bachelor’s degree preferred.
• 5+ years of experience in services for the elderly or social services in healthcare preferred.

Responsibilities

• Connects residents to supportive services and assists with transitions to higher levels of care when needed.
• Monitors the frailest residents through regular contact and post-hospital/therapy transitional care.
• Develops and maintains a resource guide, and produces monthly service reports and HUD reporting.
• Creates communication tools and liaises with resident groups.
• Coordinates socialization opportunities, including events, classes, spiritual resources, and vocational/educational programming.
• Works closely with management to ensure residents meet lease obligations and HUD requirements.

What You'll Do:

• Connects residents and home-based non-residents to appropriate supportive services and assists with transitions to higher levels of care when needed
• Monitors the frailest residents through regular contact and post-hospital/therapy transitional care
• Develops and maintains a resource guide, and produces monthly service reports and annual HUD reporting
• Creates communication tools (newsletter, website, social media, marketing materials) and serves as liaison to resident groups
• Coordinates socialization opportunities, including events, classes, spiritual resources, and vocational/educational programming, to reduce isolation
• Works closely with management to ensure residents meet lease obligations and remain in compliance with HUD requirements

What You'll Need

Required:

• High School Diploma or GED
• 3+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting
